Release of Information (FERPA)

The Family Educational Rights and Privacy Act (FERPA) affords certain rights to students concerning the privacy of, and access to, their financial aid records.

Students may choose to complete and submit the form below to the Office of Student Financial Assistance allowing the release of their financial aid records to specified third parties.

Please note that while this form authorizes MHEC to release financial aid records to third parties, it does not obligate MHEC to do so. MHEC reserves the right to review and respond to requests for release of information records on a case-by-case basis.
