Care Coordination Program Description
The Certificate in Care Coordination is designed to prepare registered nurses educated at the baccalaureate or higher level for leadership roles in Care Coordination and Case Management.&nbsp; Two of the courses within the Certificate already exist and two new core courses have been developed specifically for the certificate. Care Coordination (CC) I is a lecture course offered in the classroom and CC II is a combined in-classroom lecture and clinical course. The clinical component of the course will use a precepted model of education. <br> <br>Students will be placed with preceptors involved in care coordination and case management in a variety of health care settings. Intended learning outcomes include: Preparing Community Public Health nurses for leadership roles; Preparing Post Baccalaureate nurses to coordinate care for clients with complex health care needs as members of inter-professional healthcare teams; and Preparing Post Baccalaureate nursing students for success on the case management certification examination.&nbsp;&nbsp; <br> <br>The 12 credit hour program seeks to enroll six full-time students in its initial year and projects enrollment of six full-time students by its fifth year. <br>
